Ok so heres my opinion,
In the end he asks chuck whether it was better to live like a monster or die a good man?
Those words are what made me understand. Or at least come up with my opinion. I think,
he really is Teddy and hes not crazy. I think that the lady in the cave was real and that she worked there before they made her a patient and knew what was going on just like she said. So she told him and he knows thats hes really just being set up to be used as a test dummy, like the lady said. He was probably picked for that because he could eaisly be set up to look like he was crazy, his wife died and he served in war. So he was an easy target. Well the people who all worked there, including his "partner" who was set up to follow him and to in the end use as another mechanism to make him think he really is crazy once they tell him that. All along though, they really are giving him drugs like the lady in the cave said and that really is making him hallucinate, he's hallucinating about the things they've told him though, not things that actually happend. So when they get to the end and are trying to convince him that hes crazy, he plays along. Knowing that he is now going to be deemed as crazy because they have set him up. He plays along. He gives them what they want. The line "is it better to live like a monster or die a good man", is referring to the fact that if he gets out of there, because they have set him up, hes going to look crazy no matter what to other people because that has what he has been deemed as, an insane patient. So then its " or die a good man" meaning that he would rather die knowing he did nothing wrong and being a good man, than going back out into the real world and being treated and having to live like hes crazy. So thats why he calls his set up "partner" Chuck again so that they can say he relapsed and give him the labotmy, therefore making him their expierment and killing him. He would rather be killed knowing he was a good person and didnt do anything or kill anyone, than like I said, living back in the real world and being deemed insane because thats what the workers there are going to tell people.
