North Korea’s Role in Nuclear Proliferation |
Amid all the controversy surrounding North Korea’s nuclear programme the one clear fact to emerge is that the Western media comprehend little or nothing of the country. Yet the vulnerability of South Korea and the continuing belligerent attitude of the North make it essential to halt Pyongyang’s nuclear weapons programme. It is not only North East Asia which is threatened but the future of nuclear non-proliferation globally.
North Korea is routinely portrayed as a pariah, hermitic state with a penchant for international piracy, acting with no regard for other countries’ legitimate security concerns. Yet frequently such assumptions rest on nothing more than a bed-rock of theatre and unexamined assumptions. Too often the North’s decision-making is seen as a record of political burlesque, the proper complement to the paranoia of its dictator. This, in turn, has stimulated much uninformed speculation concerning North Korea’s goals. Perhaps, it has been suggested, the impoverished communist state is using its nuclear programme as a bargaining counter to secure aid or to ease sanctions. Alternatively, it is suggested, the nuclear programme is to be seen as part of some sort of obscure power struggle in Pyongyang.
Consistent Policies
Kim Il-Sung himself, however, is known to have worked for Soviet intelligence for many years previously.
It was the Soviet Union which created and trained the North Korean Army. In 1990 during the period of glasnost, Moscow confirmed that it was Stalin who had personally approved the plan to attack the South, an invasion which began on the night of 24/25 June 1950. The Korean War, launched with Moscow’s support and covert participation, ended in 1953 leaving the country permanently partitioned. Ever since then there has been an almost constant propaganda war replete with border incidents and punctuated by occasional acts of war. Most spectacularly, in 1983, for example, North Korean agents in Burma murdered 17 members of a visiting South Korean delegation, including four Cabinet ministers. The directives were thought to have come directly from Kim Jong-Il, currently holder of supreme power today.
North Korea’s status as an independent maverick state has largely been built up around its claim to have astutely manipulated China and the Soviet Union in the Sino-Soviet dispute to its own benefit. For example, during the 1960s there is supposed to have been a worsening in relations between Pyongyang and Moscow with the North leaning towards China in the Sino-Soviet dispute. North Korea allegedly became more receptive to Soviet overtures in the late 1960s because of the pressing need to modernise its arsenal. The Soviet Union was thus able to justify its extensive modernisation of the North Korean armed forces on the grounds of fending off a challenge for influence from China.
Yet for all its burgeoning “rogue” status North Korea is known to have conveniently helped the Soviet Union in many of Moscow’s more controversial operations against the West. North Korea provided arms and training to anti-Western terrorist groups such as the Japanese Red Army without tarnishing Moscow’s image. It has also played an important role in sponsoring terrorism worldwide and in destabilising Western interests. The principal benefactor has almost invariably been the Soviet Union. North Korean scud missiles too found their way to then implacably anti-Western states such as Iran, Syria and Libya, almost certainly with Moscow’s approval while saving Moscow from the resulting international opprobrium. Thus in the past, North Korea’s behaviour has often effectively complemented that of the Soviet Union, Pyongyang’s rogue status enabling Moscow to wash its hands of highly dubious actions. The Sino-Soviet split has provided a convincing screen for the North’s claim to independence of action, Pyongyang claiming to have skilfully played one great power off against another.
It is worth recalling at this point that North Korea’s nuclear scientists learned their trade in both Chinese and Russian laboratories. It is not a home grown programme. To the contrary, a review of Pyongyang’s nuclear programme illuminates close relationships between North Korea, China, the former Soviet Union and the successor Russian state.
The Sino/Soviet divide
North Korea’s apparent brilliance in consistently playing China off against Russia over the last half century may not be quite what it seems. The Soviet defector, Anatoly Golitsyn, in his remarkable work New Lies for Old (London 1984), has presented striking inside evidence that Sino-Soviet friction was a phenomenon of 1950-1957 which was largely successfully resolved by 1957. Later ‘evidence’ of a continuing dispute, Golitsyn alleges, was deliberately contrived propaganda designed to mislead the West and to serve Soviet and Chinese mutual interests by building up an image of dispute while forging unity of action in secret.
So, for example, Moscow’s detente overtures during the 1970s gained more credibility when contrasted with Beijing’s implacable hostility at that time. China too secured economic concessions from the West because of its apparent hostility to the Soviet Union. Worldwide communist objectives were being achieved more expeditiously, Golitsyn maintained, by the practice of the two leading Communist powers adopting dual foreign policies in apparent opposition to one another than by pursuing a single policy in open solidarity, a policy which would have provoked greater Western cohesion and resistance.
There is too much in Golitsyn’s arguments which can be more than touched upon in an article of this sort but Golitsyn makes four general points in support of his thesis that the post-1957 Sino-Soviet dispute was faked. First, frontier incidents in remote districts such as the Ussuri River, though apparently spectacular evidence of hostility could easily have been staged particularly as means of co-ordinating action between the two “opponents” were readily at hand. Second, verbal polemics were intermittent as well as pointless, suggesting they were co-ordinated rather than spontaneous. Third, despite the vehemence of the polemics, the split never reached the stage of a breach in diplomatic relations as did the Soviet-Albanian dispute in 1961. Nor was the Sino-Soviet Treaty of Friendship, Mutual Co-operation and Assistance revoked. Fourth, the hostilities can be correlated in timing with important Communist initiatives or with the beginning of major East-West negotiations such as SALT or with the visits of Western statesmen to the USSR and China.
While such an analysis goes against accepted judgement there is certainly an abundance of evidence to support it. If true then once again North Korea’s pariah status, especially since 1957, must be viewed differently. As has been shown the “Sino-Soviet dispute” has served as a useful pretext for Moscow’s provisioning of North Korea with a constant flow of military hardware. Indeed it has been one of the most illuminating examples of how duality in Sino-Soviet polemics has been used to conceal the nature of their shared goals and the extent of co-ordination in the Communist world while re-assuring the West.
It may be objected that such issues can no longer be relevant following the demise of the old Soviet Union and China’s opening to the West. Yet there are many signs that Moscow’s foreign policy is being conducted along similar lines to those of the old Soviet Union. China too still brooks no challenge to the ascendancy of the Communist party. Moscow has continued to supply North Korea with important weaponry in violation of its much publicised pledge of 1992 not to do so. Strangely, efforts to isolate North Korea at the UN have effectively been stymied by Russia and China, preventing the formation of a united international front against North Korea’s nuclear ambitions. Rightly, several observers have remarked that sanctions against North Korea can not work because China would ensure they are circumnavigated. Indeed the Chinese representative at the United Nations has called for more tolerance and understanding towards North Korea, a particularly arresting call in view of Beijing’s usually wholly intolerant, even brutal, attitude towards dissent.
The End of Nuclear Non-Proliferation
North Korea’s pariah and maverick status can therefore be validly re-examined for the possible relevance they have to Moscow’s and Beijing’s long term political and strategic aims. North Korea is threatening to wreck the Nuclear Non-Proliferation Treaty. This is an especial concern of the United States. Washington’s principal concern is not a direct nuclear attack from the North but that the country will export its nuclear technology to other rogue states or terrorist networks, a very real possibility. After all, North Korea has already acquired billions of dollars by covertly exporting its missile technology to the Middle East. Pyongyang holds terrorism to be consistent with North Korea’s revolutionary role and so it can not be ruled out that Pyongyang will take an equally irresponsible attitude to the risk of spreading nuclear destruction. They have certainly behaved as if they were ready to take their country to the brink of war as was evidenced recently with their unilateral abrogation of the 1953 armistice. It is hard to see what can be done about this in view of the tacit practical support given Pyongyang by both Moscow and Beijing whatever their public statements may say to the contrary.
Collective Security Arrangements
One need look no further than the cynical polonium poisoning of Alexander Litvinenko in December 2006, an action which must have required high level clearance in Moscow, to see how cynically the Russian leadership views the use of nuclear materials.
Co-incidentally it is precisely in the area of nuclear proliferation where Russian spokespeople highlight the failure of existing international policies. Natalya Timakova, chief spokeswoman for Prime Minister Dmitry Medvedev said of North Korea’s latest nuclear test: “(it) deals a blow to international efforts to strengthen the global regime of nuclear non-proliferation.” Medvedev himself said recently: “Non-proliferation is one of the most important areas where Russia and America can work together.”
North Korea’s cynical assault on the Nuclear Non-Proliferation Treaty threatens to subvert nuclear arms control at a critical moment. It is taking place at a time when Moscow is pressing the West to accede to elaborate “collective security” arrangements which Moscow holds are now essential to prevent chaos breaking out with the wholesale proliferation of weapons of mass destruction. Yet, as we have seen, there is significant evidence to suggest that it is Moscow itself and its satellites who are behind this proliferation. The likely regional solutions, which are being canvassed by Moscow, involve a significantly changed new world order. In Europe itself it may spell the end of the British and French deterrents with the passing of power to Eurasia’s sole military superpower, its strength based firmly on nuclear blackmail.
Distracted by the North Korea’s maverick status, it has been hard for Western analysts to see that much of North Korea’s behaviour is part of a planned and cumulative international strategy aimed at the steady incremental expansion of Moscow’s and Beijing’s influence in their respective regions. Until the West abandons its simplistic thinking about developments in the former Soviet Union and North East Asia; and faces the very real probability that these events are not random coincidences but part of an unfolding strategy, the misinterpretation of events will continue with potentially highly damaging consequences for the world’s democracies.

Banking Restructuring ? Lessons for Georgia |
Restructuring: concept, goal and contest. Termini restructuring is of Latin origin and means changing-improvement of the structure of some object or system, i.e. its forms and consistence (morphology). It basically means unchanged character of directions of its functioning. They use restructuring from large plan in the economical texts mostly with debts, including foreign ones, payments and taxation (trade) balance, corporation sector of the economy and of separate enterprises, of banking system entirely and separate banks (other credit organizations).
They define restructuring in legislation in the following way: restructuring of credit organization is a complex of activities directed towards eradication of financial fluctuations of the organization and recovering its pay abilities or towards realization of liquidity of this organization. This definition doesnt make needed opinion about the occasion to be discussed, as, in the first place, here they mean only separate credit organizations and not banking system itself, and, second, it has very technical character and mixes the essence and contest of the process of restructuring with the activities, which may (or must) be realized in this process.
Thus, there is not common, widely excepted definition of restructuring, though majority agrees with the idea, that we must consider restructuring to be readjustment of (cure) of banking system and its taking out of the crisis phase, also its returning to the conditions of good labor abilities. They sometimes use termini of stabilization of banking system, but we consider it to be comfortable. The fact is, that achieving stability may be provided in various ways, including the one of liquidating whole system. There is another point of view, that they consider restructuring to be the process for overcoming difficulties, appeared during the crisis. This point of view is not quite fluent.
Thinking of the essence of the affair and not its definition, hen we must consider in reconstruction of banking system as a process totality of decisions and actions. Its basic elements are:
eradication and minimization of negative influence of bad macro-economic, political and other common factors upon situation and perspectives of functioning of banking system; improving systemic organization (structure, kinds, types) of totality of credit organizations, creation of conditions for effective and civil competition among them; improving legislative base for mutual-advantage collaboration and organization-economic mechanisms among credit organizations and their clients; increasing quality of managing entire banking system and its separate elements; financial cure of separate banks and other credit organizations; effective (with minimal social experiences) liquidation of vital credit organizations.
Foreseeing these elements, we can state following definition: restructuring banking system in managed process of its global readjustment (improvement), supported by changing in industrial, cash, taxation, budgetary and information policy, also in the policy of the banks themselves, and which is directed towards formation of banking system adequate to effective, trustful and dynamically developed modern requests.
According to this definition, restructuring effective, stabile and healthy banking system in not needed (though, it is possible to improve or reform it). Thus, restructuring is a cure (curing something that is not healthy), i.e. restructuring may be understood and must be understood to be the process, with the help of which banking system of concrete country transit to the new level of development. It is also evident, that restructuring is curing of such systems, which are in crises and can not get out of it without help. Finally, from the point of restructuring (privately displaying necessity of financial curing) we must discuss absolutely every bank. In this case, restructuring, as a process of readjustment, seems to have its own instrumentation, which will not be bounded only with the instruments of ordinal procedure banking management?
According to the mentioned above, we can make main goal of restructuring process of banking system its recovery and taking its movement to the relatively new trajectory, at which it already gains earlier lost potential of progressive development and becomes adequate to the real sector of the economy again.
In relation with this, we must pay attention to the following principle requests towards the context of the process to be discussed: activities provided in relation to the restructuring will be profitable only in case, if we foresee not only reasons of banking crisis, but also define those fundamental; defects of economical relations, which make banking system viable at the modern stage; restructuring of banking system, which, in fact, must give rise to its reanimation in the earlier condition, doesnt solve problems neither of whole system, nor of the country economy; it is necessary to process not only tactical activities before starting the process of restructuring, but also to set strategic objects: to receive such structure of banking, which will be adequate to the goals and functions standing towards the banks at the new stage;
while processing activities of reforming banking system they must clearly define a circle of those problems, which must be solved during the process of reforming with the help of renewed banking system and they must set the price of activities;
effective restructuring requests combined methods of approaches towards the problems. World practice processed principles and methods of approaches of solving banking crisis, approbation of which showed their sufficient effectiveness. It is nonprofessional and not expedient to use some principles and refusing others;
a process of solving crisis may not be fast, simple or cheap.
Th8is common goal, mentioned above, in its turn, may be concreted into the list of those problems, working at which must form real concept of restructuring process according to the conditions in modern Georgia:
eradication of conditions provoking banking crisis, solving problems in relation with banking sphere and real sector; financial curing of those problematic banks, which have kept viability and perspectives of development, also state support of those banks, which have abilities of effective usage of this aid; providing trustful satisfaction of basic current requests on bank services (payments and short-termed crediting) of the industrial subjects; foundation of a new, more complete structure of banks and other credit organizations (according to the forms, measures of the property, regional distribution and so on); Creation of more complete rules and instruments regulating new rules of banking activities and of this activities; Creation of conditions. Mechanisms and stimuli for turning banks to the side of enterprises, for their involving into the process of further production, also overcoming inflexibility of the banks in the process of solving investment problems; Recovering trustfulness among banks; Recovering trustfulness in relation with the banking system, appearing stimuli among population for putting their savings on the accounts; Creation of the stimuli for increasing responsibilities and effectiveness of the bank managers; Civilly closing of not viable banks and fulfillment of the mechanisms of their liquidation.
There is an idea about the fact, that main goal of restructuring banking system is recapitalization of the banks (recovering lost a capital and its further growth), but it is not quite correct: since today the hardest problem is, that a spectra of profitableness and trustfulness of capital investment is very tight.
Some bankers offer such understanding of restructuring and such pragmatic activities of radical reforming of banking system, the essence of which finally has been brought to the regrouping of the almost bankrupted banks according to the principles of specialization (specialized banks working in the country scale, banks oriented towards export or those obligated in the groups of large enterprises, also regional banks). they meant, that new system forming groups would obey to the strict control of appropriate governmental structures or groups of enterprises, in exchange of it, it will have right for working on budgetary resources. Suggestions of separate bankers were not related with the problems of recovering whole banking system.
We can form basic problems of restructuring banking system in the following way:
Transiting to the foundation of a healthy market banking system by readjustment of separate problematic banks, providing structural reform of banking system;
Increasing whole capital of banks and filling banking system with long-termed resources;
Creation conditions stimulating growth of the quality of market commercial banks, including those in the regions.
Main goal of restructuring program must be: creating such layer of technological market commercial banks, which provides marketing policy and makes basic profit from credit-operations. It is interesting, that within the bound of 2-3 years program share of such capital in the banking system may reach up to 30-40%, and credit share in the credit portfolio of banking system 30%. Share of profit made from crediting in whole income of banking system must not be less, than 6%. Half of such healthy banks must still function in the regions.
The concept of those first steps, which must make foundation to the realization of effective program of restructuring banking system, must be formed in this way:
processing a conception of developing banking system and its taking as a manual 2-3 years earlier; consisting a program on working at the passed liabilities and its realization;We mean that a special state organ working on restructuring purchases from the banks their bad money;. realizing recapitalization of the banks. we mean, that the same or other state organ enters the capitals of the banks for a little time, increases this capital and credit potential of the banks and then sells its share in the capitals of the banks; passing the law about guaranteeing deposits of the people in the commercial banks on time; creating equal conditions for competition and development of every bank. We must refuse other banks having social importance, forming a system and somehow related with somebody; clear formulating of the role of government and state banks in the banking system; realizing evident support of regional market commercial banks; creating effective mechanisms of developing infrastructure of the banks and their functioning; changing a system of taxation of commercial banks; processing and realization of activities refinancing commercial banks by the central bank; creation and realization of the package of legislative and organization activities developing hypothec crediting; fastening the process of transiting commercial banks to the international standards of accounting
Restructuring: principles and conditions. We can name following to be the obligatory principles (main rules) of the process restructuring banking system:
A principle of solidary obligations. The essence of it is, that in the mentioned process there participate (with out resources) and coordinate the banks themselves (in the first place the owners), their creditors and the government. It is impossible to restructure banking system without state support. Though, it is evident, that the state will not be able to support every bank, having extremely reduced resources. Accordingly, the banks in the first place must try to solve their problems independently and the managers and creditors of the cured banks must stand on the advantage position.
The principle of minimizing loss and expenses. It says, that while realizing restructuring we must consider those activities, which give the opportunities of overcoming crisis with the less budgetary expenses (financial expenses of the society) and with little loss from the side of banking system and the clients of banks to be more prior.
Liquidation of problematic banks is much losable, but socially more difficult way. It needs especially measured method of approach towards the problems of the depositors the banks to be liquidated. Fast liquidation of not solvent banks may deepen the crisis (an Indonesian example in 1997-1998). According to the estimation of numbers of experts, best way out of it is confluence of the problematic bank with the healthy one, though this is quite doubtful recommendation.
A principle of minimizing liquidation requests to give priorities to the activities of reorganization and support and not bankrupting in the process of restructuring and financial curing.
A principle of just distribution of expenses on restructuring mean, that the stated part of expenses on curing banks must be compensated by those, who receive risks related with these banks, are responsible for their loss and make profit after restructuring (i.e. participants of the banks, its highest administration). Economical obligation of not solvent managers and owners of the banks may be expressed, for example, by adequate reduction of their own banking capital, their participation in the restructuring process in the way of additional entering in the bank capitals. Part of the loss may be covered at the expense of the depositors.
A principle of strategic method of approach means definition of the strategic problem, what kind of banking system is wanted by the society after restructuring (in the condition if it conforms to the new purposes and functions of the banks at the new stage). Only after this they must select advantage and agreed activities, which may be recommended for readjustment of separate banks and its entire system.
A principle of complex method of approach means, that a system defined by the program must be fulfilled completely. It is impossible to bring whole concept of reconstruction process to its separate consisting parts (for example, everything mustnt be bounded only with solving financial problems displayed in one concrete period of time).
We can name such principles of providing restructuring, as transparency (necessity) of distributing expenses related with it, strengthening management of those banks, which are supported by the state, encouraging independent adaptation of the banks with the changed situation and others.
Following conditions of restructuring are also of great importance:
Success of reconstructing banking system is in close touch with how much clearly and knowing affair they formulate long-termed problems, industrial, structural and financial policy. For example, crediting enterprises, especially of large ones, is possible only in the case, if it is effective, if it leans upon clear state strategy. It is an important condition without which bank restructuring may not be successive. For successiveness of restructuring banking system it is the most necessary to recover and develop such conception, which will conform to the main economical goals. This conception may be oriented towards fulfillment of objects of operative character (recovering a mechanism of payment, solving other problems). Its not real to try restructuring banking system, apart form reforming and restructuring the surrounding, in which bank functions. This request in the first place touched upon real sector of the economy and its active enterprises. Banking system is a kind of built and its successful restructuring is able by knowing what the basis is like, i.e. what kind of economy shall it support. Concrete requests and possibilities of real economy are the main criteria defining, what the banking system must be. Solving problems of real sector requests advantage economical policy of the state. In every case, real sector and banking system also needs concrete orienteer, which themselves appear to be additional stimuli of development. Restructuring of banking system must be outrun by curing of money and credit relations. Here curing of money relations must play leading role (privately avoiding barter and other cash-free payments in the economy). This needs providing activities stimulating investments in real sector of the economy, because it is impossible to come out of the economical crisis in other case. Restructuring banking system is impossible without staff revolution. Specialists of short-termed financial speculations must master either modern directions or give the way to those professionals, who can work with the real sector, have knowledge and ability in the part of estimating and managing investment and industrial risks. Restructuring banking system requests important correcting of legislative and normative base.
According to the mentioned above, we can separate some leading questions about restructuring banking system, on which there still is no satisfying answers.
What was the main reason of the situation, in which the banks appear? Its thoughtful, that we must make choice among situation in real economy and peculiarities of bank activities. We consider it not to be correct to oppose two reasons to each other this way in our concrete conditions. What must turn into the basic concept of restructuring process (important consisting part)? Managers of central banks give different answers to this central question. There are such variants not conforming to each other: 1. financial curing of those problematic banks, which keep viability and perspectives of development; 2. rising the level of capitalization of the banks; 3. curing money relations; 4. recovering not as much of banks, but cash-credit relations; 5. creation functionally new banking system. What must the structure of banking system be, formation of which is purposeful? This is a principal problem. It is necessary to clear everything related with this. The banks must have an assortment if there is no other variant to be received. What must a conception of recovering and developing banking system be? Privately, following questions still remains to be doubtful (more in practice, then in theory): must we consider basic direction of curing banks their reorganization and readjustment, which concerns financial and other support and structural reforming?
Foreign experience of restructuring banking system. Bank reconstruction is not a unique problem. Banking crisis has been noticed almost in 70 countries during 20 years. A process of recovering balance has been continuing very difficultly everywhere and the state participated in them (though, scales of this participation were different in the different places and periods. Sometimes reasons of the crisis coincided with each other, sometimes they were specific. But forms of their solving coincided in many cases: stabilizing crediting, filling own capital of the banks, purchasing their assets (including passed debts) and others. As a rule, basic financial heaviness leaned upon the state directly, or in the way of financing specially founded agencies by it.
USA was a pioneer in the field of banking restructuring, where a system of guaranteed deposits and a special institute managing these deposits has been founded under the influence of the crisis in 1929-1933. This institution was a federal corporation insuring deposits (FCID).Next stage of restructuring banking sphere is related with the series of banking crisis took place in absolutely different countries during last 20 years.
In 1980-1991 1300 banks and 1400 borrowing-saving associations stopped existence in the USA. According to the different estimations, restructuring banking system cost 300-500 billion dollars (5% of the WIP). In 1995 banking crisis took place in Japan, in 1994-1995 in France, in 1989-1990 in Australia, in 1987-1989 in Norway, in 1991 in Sweden, in 1991-1993 in Finland, in 1980-1982, 1900-1991 and 1995 in Argentina, in 1990, 1994-1995 in Brazil, in 1981-1982, 1990-1991 and 1995 in Argentina and Mexico, in 1982-1984 in Chile, in 1994-1995 in India, in 1994 in Indonesia, in 1985-1988 in Malaysia, in 1981-1987 in Philippines, in 1991-1995 in Hungary, in 1990s in Poland, Bulgaria, Lithuania, Latvia, Estonia and others. In some countries systemic crisis used to be repeated periodically. Some countries were able to avoid systemic basic crisis with the help of having insurance system, in the first place, at the expense of effective banking management and regulation.
Price of restructuring banking system is very different: 5% of WIP in the USA, 10 in Hungary and Brazil, more, then 40 in Chile and 55% in Argentina.Central banks can support problematic banks in the crisis situation, especially in case of spoiling their current liquidity. In Venezuela, eight not solvent banks used special lines of liquidity for compensating money resources. Though, they were not able to cover borrowed sources in the future.
In other cases, crediting is an important step of central banks. They are supported during banking crisis and give them resources and terms for restructuring credit organizations. A long termed support provided by the central bank of Poland is a good example of it, when it purchased low profitable shares and long-termed bonds from the banks. Granting long-termed credits by the central bank sometimes depends on creation of complex plans of improving situation by the banks (list of stated activities and expected results).
Reducing the level of obligatory reserves (or increasing percentage payments on them) is another way of supporting banks, for example, part of obligatory reserves of deposits poste restates were set free for financing purchasing certificates of termed deposits of the institutions, which had been working by the program of restructuring banks.
They use special tax advantages very seldom in the process of bank restructuring. Notwithstanding this, Brazil used tax stimuli for encouraging confluence: swallowed bank could exclude then value of not active credits, the sallower received credits equaled to the distinction between the purchase and balance prices. Some countries use tax stimuli for shares and bonds issued during the realization of restructuring program.
They somehow make the rules of regulation and management under the conditions of restructuring banking system simple. They compensate this by creation such middle-termed system of regulation and management (in the crisis period), which foresees risks of banking activities more adequately.
To save the banks being in hard position actions provided by the state may support weakening the feeling of responsibility of the banks. In such conditions, the following is of great importance not to give rise to the weakening encouragement of irresponsible behavior of the banks in the future. It is considered, that it is necessary to grant a sum for making large profit and participants of the banks must be responsible for their obligations. They requested from the banks to discard a capital partially a conditions for making support in South Korea; the state obliged itself with bad debts of credit organizations in Mexico only in case if its participants used to make additional income; while bankrupting of credit organizations in Brazil and India, their participants were obliged to enter additional sum equal to the size of their initial entering in the nominal fund.
Herewith, participant of the banks are not always obliged with the responsibilities, for example, in case of the loss received from those credits, which are granted by banks by the state indications, it is necessary to range the size of responsibility, as the participants may not have possibility for solving problems in the credit organization because of the not having transparency and of the organization calculations and other reasons.
They founded specialized institutions in the most part of the countries during restructuring banking system, which have been obliged with the problems of managing this process.
A government and central banks of many countries solved problems with bank crisis and restructured own banking system in different ways. Practice has shown up, that there is neither ideal form of restructuring, nor the universal strategy of normalizing situation in banking sector. Very often this or that action depends on concrete occasion. Notwithstanding this, we may separate total signs of successful programs, which have been realized abroad:
the fastest definition of the scales of problems, its recognition on the state level and readiness of the government for granting important financial resources for solving problems;
passing transparent, activities adequate to the essence of the problem, moving “bad” assets from the problematic banks off;
processing complex, transparent, operative program, its correct and successive development;
fulfillment of the procedures of banking management.
Chile. A complex of activities. Large scaled restructuring of banking system in Chile has started since 1984, when a central bank of the country started granting stabilizing credits for supporting liquidity of the banks and purchasing their not trustful credits or changing not active assets on liquidity. Deregistration of the bank debts took place in the way of turning creditors into the shareholders.
State became a guarantor for foreign debts of the private banks. Size of the debts transited to the central bank of Chile by the end of 1985 overcame whole capital of problematic banks 3 times and consisted 6 billion dollars (25% of WIP). About 60% of expiated credits were changed on its bonds by the central bank.
They involved straight state control in numbers of system forming banks of the country.
Recapitalization of the banks transited under the state control used to be realized in the way of additional issuing of the shares placed among small and middle investors. A state corporation of supporting development (CORFO) worked on this program.
Results. The banks practically fulfilled their obligations in the part of the deposits of physical and juridical persons with the help of the used activities. Though the quantity of national private banks was reduced from 22 to 15, but they were able to keep every large bank and improve their working by 1987. After 1996 every commercial bank of Chile has been considered to be competitionable.
The value. By the end of 80s, expenses provided on restructuring banking system of Chile consisted from 30 to 40% of the country WIP.
Mexico. A complex of activities. Restructuring of banking system in Mexico has been started in 1995 after devaluation of national currency and strengthening of financial situation, which followed devaluation. One of the nominal activities of Mexican bank (main bank of the country) was involving special calculation unit UDI (unidades de inversion), which was indexed with the level of prices. Whole assets of the banks were calculated by it, for avoiding devaluations of credit portfolios of the banks.
Basic organ working on restructuring banks was banking fund of protecting savings (FOBAPROA). During the process of readjustment it expiated securities from the bank and banks provided deposing of received resources in the Mexican bank. They gave five years to the banks for expiating these papers. In other cases they were going to convert them into the shares of these banks and their realization at the market.
For solving problems with foreign debts of national banks central bank granted short-termed currency credits from these banks to them, who passed payment of these obligations. At the same time FOBADROA published doubtful assets of commercial banks. Herewith, the shareholders, in its turn, were obliged to enter sums equal to the half of resources granted by FOBAPROA. Banks were to enter resources received from this operation into the 10 year bonds.
They involved outer management of problematic banks. In some cases they granted their shares to the foreign banks. At the same time they supported bank debtors, provided restructuring of their liabilities.
Results. With the help of restructuring Mexico could avoid destroying of banking system. Deposits of the people practically were not defected. They kept trust of foreign investors in the banks of the country.
The value. Restructuring of banking system of Mexico costs 60-65 billion dollars (about 14.5% of national WIP).
Argentina. A complex of activities. State governmental organs in Argentina supported more trustful banks for solving problems of banking crisis. They used differenced method of approach. They separated banks into several groups:
I group – Middle and large banks having temporal problems of liquidity because of loosing clients. They granted them credits of central bank of Mexico and Banco de la nacio Argentina;
II group – small banks, which were to confluence with large, relatively healthier banks or were to be swallowed by them;
III group – small banks, which were at the edge of bankrupting. They stopped operations in these banks and they desired to readjust, sell and liquidate them;
IV group – 12-15 state banks owned by the administration of Argentinean provinces, which were to be privatized.
Expiation of “bad” debts was provided not by state agencies, but file largest commercial banks, which founded a special trust-fund in January 1995 together with the central bank. For compensating provided expenses they reduced a normative of reserved requests.
Results. A complex of anti-crisis activities in Argentina provided stabilization of banking system in one year. Though during this period position of foreign banks were significantly strengthened. There share in the total assets of banking system of the country drew up 42%.
The value. Expenses wasted on restructuring banking system of Argentina drew up 2.5-3.5% of the country WIP according to the various estimations.
Scandinavian countries. A complex of activities. notwithstanding distinctions in the reasons provoking crisis in every Scandinavian country, a conception of restructuring banking system leaned upon practically united conditions and principles of state support. They are:
founding a special state organ for overcoming banking crisis and providing state support avoiding mediator agencies; strengthening the role of prudential banking management and separating a special organ for its realization; overcoming deposits in relation with as physical, so juridical persons with the method of joint approach (“nobody looses”); supporting every bank in equal principles and conditions and fluent transparency of information for entire society; total method of estimating situation in banks and displaying need of state support. This support a confluence with usage of “not popular” activities towards owners and managers, which supported them. These are such sanctions, as compensation of material loss at the expense of the property of guilty managers of the bank, devaluation of nominal capital until loosing whole resources (this activity in Norway was obligatory) and so on.
Programs of restructuring banking systems of these countries were being realized by optimal conformity of responsibilities and stimuli at every level including owners, organs of managing separate banks and regulating organs of whole system.
A value. Expenses in Sweden consisted 4.3% of WIP and 9.9% in Finland.
Hungary. A complex of activities. Restructuring banking system has been continuing in two stages. At the first stage (1992-1995) the state used to transfer important sum of money in problematic and, in the first place, large banks. At the same time clearing of bank assets in the way of changing “bad” credits at state bond took place. They used state guarantees very actively. They tried to use special structure for centralized restructuring – capital investment and development bank of Hungary, but by 1995 the banks appeared again under the critical situation.
At the second stage (1995-1997) method of approach in relation with restructuring has been changed. They used state resources only for supporting those large state banks, which were to be privatized. Restructuring made them attractive for western investors.
Results. State share in the bank capital after first stage has been increased from 41.4% in 1991 till 69% in 1994. The result of second stage was increasing the share of foreign capital in the banking system of the country till 60% by 1997 (in 1996 – 48%, in 199535%, in 1991-1994 – 12-15%) and reduction of the share of state property till 20.66%. Share of “bad” debts in total sum of debts by 1993-1997 has been reduced from 13.2 till 1.2%. There was no occasion of selling deposits or refusing returning money to the debtors.

Creation regime of prior support for restructuring banks.
For the purpose of widening possibilities of postponing own capital by the banks:
They abolished prohibition on paying entering in the nominal capital of the banks and receiving subordination loans by the foreign capital;
They processed a rule of making entering in the nominal capital of the banks, when it takes place at the expense of converting bank obligations in the licenses of participating in the nominal capital;
They changed methods of approach towards requests of minimal size of own capital. These requests are still active for newly founded banks and are abolished for the active ones;
The involved the rule of paying nominal capital of the banks by the state securities.
By the end of 1999 Russian bank announced finishing of the first stage of restructuring banking system, which means, that this system recovered ability of providing basic complex of service, they kept viable kernel of the banks.

The state refused to support practical programs of restructuring banking system as well. After these, the society suffers more and more loss.
They often call a process of transforming banking system in Russia “slowly progressive restructuring”. Experience of overcoming banking crisis in the USA. US economy has been influenced by banking problems several times. One of them appeared during the period of large depression of 1929-1933. Thousand of bank deposits were devaluated because of ineffective operations of the banks, also not returning of the granted loans and total degradation of the economy. They closed hundreds of banks and provided confiscation of thousands of objects because of not returning of loans. US congress passed laws about founding federal corporations insuring deposits and also those insuring loans and savings. Except insuring, these corporations were obliged to research a lot of financial institutions for the purpose of finding defects in bank legislation and procedures and regulation of banking operations. Securitization of commercial loans united in pulls (emission of turnover securities by provision); Wholesale of the rights of request on credit to the large market corporations; Concluding open auctions in the regions; Organization of conducting open tenders on the federal level; Conducting auctions and tenders by participation of foreign investors; Realization of immovable property by the scheme of financing agreement; Creation data base of the objects of immovable property managed by RTC (the base became available for the interested buyers allover the world).
RTC administration knew, that mission of this corporation would be hard to be fulfilled, without having correctly planned strategy, strictly processed methodology and procedures, also without strict control. The most difficult problem was conforming conditions of selling assets, covering loans and restructuring, also stating responsibilities and limits of wasting RTV resources.
They found way out by setting limits and wide distributing obligations. The procedures of transiting obligations to the staff workers foresaw preparing statement on every asset.
As a rule, main reason of banking problems is bad management. One thing I would object strongly to, is the idea that cystic acne is a symptom of western diet. In fact, those of us who have travelled may have noticed that traditional Asian and African diets are no better for their culture than western diets, at least in terms of the incidence of cystic acne amongst their people. There may well be strong evidence to suggest that western diets are not actually very good for us but there really is no evidence that it can either cause or worsen cystic acne.
I am not seeking to totally discredit the claim that diet has a part to play in cystic acne but still there are lots of other factors that can trigger this skin condition like hormones, genetics, vitamin deficiency, stress and much more.

INTRODUCTION
Food allergy is recognized as a common worldwide problem, and, like other atopic disorders, its incidence seems to increase. In the past years, investigations of allergic food proteins and related immunological responses have moved to the molecular level, and the newly-found knowledge might provide novel experimental strategies for the laboratory diagnosis and the immuno-modulatory control of food-induced allergic reactions (1, 2).
Approximately 20% of the population alters their diet for a perceived adverse reaction to food, but the application of double-blind placebo-controlled oral food challenge, the gold standard for diagnosis of food allergy, shows that questionnaire-based studies overestimate the prevalence of food allergies. The clinical disorders determined by adverse reactions to food can be classified on the basis of immunologic or no immunologic mechanisms and the organ system or systems affected (3, 4, 5).
The true prevalence of food allergy is lower and seems to range from 1% to 4% of the general population and about 6% of the general population and about 6% of the paediatric population, but does occur in as much as 25% of children with eczema6. Allergic hypersensitivity simply put, is an adverse immune reaction to a protein (or allergen) in our environment, which is normally harmless to the non-allergic person. It may present as mild itching of the skin, tissue swelling and wheezing or even progress to full-blown anaphylaxis and death. 18 million allergy sufferers live in the UK where 4% of the populations also have a food allergy.
Allergy develops after repeated exposure to the causative allergen. Sensitization takes place on initial exposure (a process that may take up to six weeks to develop) and no adverse reaction appears to occur during initial sensitization (6).

1. Anaphylaxis: a severe, whole-body allergic reaction that can result in death. Leads to vasodilation and, if severe, symptoms of life-threatening shock.
2. Angioedema: rapid swelling (edema) of the skin, mucosa and submucosal tissues, especially of the eyelids, face, lips, and tongue.
3. Eczema is a form of dermatitis, or inflammation of the upper layers of the skin.
(a). Atopic eczema (aka infantile e., flexural e., atopic dermatitis) is believed to have a hereditary component, and often runs in families whose members also have hay fever and asthma. Itchy rash is particularly noticeable on face and scalp, neck, inside of elbows, behind knees, and buttocks.
(b). Contact dermatitis is of two types: allergic (resulting from a delayed reaction to some allergen, such as poison ivy or nickel), and irritant (resulting from direct reaction to a solvent, for example). Some substances act both as allergen and irritant (e.g. wet cement). Other substances cause a problem after sunlight exposure, bringing on phototoxic dermatitis.
(c). Xerotic eczema (aka asteatotic e., e. craquele or craquelatum, winter itch, pruritus hiemalis) is dry skin that becomes so serious it turns into eczema. It worsens in dry winter weather, and limbs and trunk are most often affected. The itchy, tender skin resembles a dry, cracked, river bed. This disorder is very common among the older population.
(d). Seborrhoeic dermatitis (aka cradle cap in infants, dandruff) causes dry or greasy scaling of the scalp and eyebrows. Scaly pimples and red patches sometimes appear in various adjacent places. In newborns it causes a thick, yellow crusty scalp rash called cradle cap which seems related to lack of biotin, and is often curable.
4. Skin rashes, such as nettle rash (also called urticaria or hives). Some of these longer lasting rashes are called atopic dermatitis.
5. Itching of the mouth, throat, eyes, skin, or any area
6. Nausea, vomiting, diarrhoea, stomach cramps, or abdominal pain
7. Running nose or nasal congestion
8. Wheezing, scratchy throat, shortness of breath, or difficulty swallowing
9. Mood swings, depression: The symptoms of an Immunoglobulin E (IgE) allergic reaction can take place within a few minutes to an hour. The process of eating and digesting food affects the timing and location of a reaction. IgG reactions build over a period of hours to days, and therefore symptoms can be difficult to notice as allergy-related (7).
TYPES OF ALLERGENIC FOOD
There are a number of groups of foods that are responsible for causing the majority of food allergies (6). Rice allergy is more common in East Asia where rice forms a large part of the diet. In Central Europe, celery allergy is more common. The top allergens vary somewhat from country to country but milk, eggs, peanuts, tree nuts, fish, shellfish, soy, wheat and sesame tend to be in the top ten in many countries (8).
The most common food allergies are:
1. Milk allergy:-
Two out of a hundred infants under one year old suffer from cows milk allergy, making it the most common food allergy of childhood. In general children lose this sensitivity as they grow up with nine out of ten losing it by the age of three; it is unusual for adults to suffer from this allergy. Symptoms are frequently vomiting and diarrhoea in children, with 30-50% also having skin rashes of some type. A small number of children have an anaphylactic reaction to milk which tends to be life-long.
The major allergens in milk are the caseins and the protein b-lactoglobulin. People are usually allergic to more than one kind of milk protein. The proteins from cows milk are very similar to those from goats and sheep. Thus goats or sheeps milk cannot be used as a cows milk substitute in allergic individuals (8).
A report about 22-year-old Female patient is described who was repeatedly hospitalised on account of severe asthmatic attacks presumably due to the ingestion of cows milk or milk-containing products. There were no signs of gastrointestinal disturbance, but some urticaria and angioedema occurred. Strongly positive RASTs (radioallergosorbent test) were observed in the blood serum against the proteins in cows milk, bovine serum, egg white, cod fish, and house dust. The symptoms were successfully controlled by rigorous dietary measures (8).
2. Eggs:-
Allergy to eggs is usually observed in young children rather than adults, and like cows milk allergy, fades with time. Occasionally children suffer from a severe form of allergy which is not outgrown.
The main allergens are the egg white proteins ovomucoid, ovalbumin, and ovotransferrin. The eggs of other poultry, such as ducks, are very similar to those of hens and can cause reactions in egg-allergic individuals (8).
3. Peanut allergy:-
Peanuts are one of most allergenic foods and frequently cause very severe reactions, including anaphylaxis. Allergy to peanuts is established in childhood and usually maintained throughout life. Peanut allergy can be so severe that only very tiny amounts of peanut can cause a reaction. Thus the traces of nuts found in processed oils, or the carryover of materials on utensils used for serving foods, can be enough in some individuals, to cause a reaction. The main allergens in peanuts and Soya are the proteins used by the seed as a food store for it to grow into a seedling. One of the allergens in Soya bean is very similar to a major allergen from dust mites, a common environmental allergen. We arent sure yet whether this means there is a link between dust allergy and Soya allergy.
A large number of children who develop peanut allergy have their first reaction the first time they are given a peanut-containing product (usually a dab of peanut butter). Peanut protein could pass into breast milk Twenty-three lactating women; aged 21 to 35 years ate 50 grams of dry roasted peanuts (about 60 peanuts or 1/3 cup). Breast milk samples were collected at hourly intervals. Peanut protein was found in the breast milk of 11 of the mothers. In 10 mothers, it was detected within two hours after she ate peanuts, in one mother it was detected six hours later.
That peanut protein is secreted into breast milk, thus sensitizing the baby who is at risk for developing an allergy. This may explain why up to 85 percent of children have a peanut allergy reaction the first time they eat a peanut-containing product. A baby born into a family with allergies.
[Note: - Milk, eggs, and wheat have previously been shown to pass into breast milk. The mothers from allergic families eliminate peanuts and tree nuts (e.g., almonds, walnuts, etc.) and consider eliminating eggs, milk, fish, and perhaps other foods from their diets while nursing. If you choose to do this, be sure to speak with a registered dietitian to be sure your diet is well-balanced (8).
4. Tree Nut Allergy:-
This group includes true tree nuts, such as Brazil nuts, hazelnuts, walnut and pecan. Whilst not as intensively studied as peanuts, indications are that tree nuts can cause symptoms as severe which can occasionally be fatal. Children who become sensitised to tree nuts tend to remain allergic throughout life. Hazelnut and almond allergies are more like those people get to fruit, and are linked to pollen allergies. Nut allergens can be both destroyed by, and resistant, to cooking and we think that roasting may actually create new allergens. The allergens can be the seed storage proteins, or other molecules which are also found in pollen.
5. Fish and shellfish allergy:-
Allergies to shellfish are unusual in children, mostly being experienced by adults. Reactions to fish are found in children and adults. The incidence of seafood allergy is higher in those countries with a high consumption of fish and shellfish. Severe reactions are more frequently found with these foods, including anaphylaxis. Cooking does not destroy the allergens in fish and shellfish, and some individuals maybe allergic to the cooked, but not raw, fish. The major allergens in fish are flesh proteins called parvalbumins which are very similar in all kinds of fish. Shellfish allergens are usually found in the flesh and are part of the muscle protein system, whilst in foods such as shrimps, allergens have also been found in the shells.
If pregnant or trying to be, avoid seafood and shellfish. Some believe that it may cause your child to develop food allergy.
6. Fruits allergy:-
In general allergic reactions to fruits and vegetables are mild, and are often limited to the mouth, and are called the oral-allergy syndrome (OAS). Around four out of ten people having OAS are also allergic to tree and weed pollens. Thus people who are allergic to birch pollen are much more likely to be allergic to apples. There allergens in fruits and vegetables are not as complicated as other foods. Many of them are very like the allergens in pollens, which is why people with pollen allergies are also allergic to certain fruits.
Many fruit allergens are destroyed by cooking, and thus cooked fruits are often safe for fruit allergic people to eat. Allergies to latex gloves, especially amongst health professionals, are increasing. As many of the latex allergens are like those found in certain tropical fruits, such as bananas, these people can get an allergic reaction to handling or eating these foods (8). Tomato Allergy: - Reported on four cases (two adults with throat constriction, one child with gastro esophageal reflux disease [GERD], one child with atopic dermatitis) with IgE-mediated reactions to tomatoes.
Using extracts prepared from the skin, seeds, and fleshy fruit of the tomato, specific IgE antibody in the sera of patients was detected by ELISA. After characterizing the extracts for their antigen profile and reactivity with IgE, proteins were separated by SDS-PAGE (sodium dodecyl sulfate-polyacrylamide gel electrophoresis). Of the four patients, one adult showed higher IgE against tomato skin extract. Western blot tests of the patients scrum with tomato skin extract also revealed reaction with a protein band in the 42-45kD region. Tomato allergy can be manifested as atopic dermatitis, laryngeal angioedema, and even gastroesophageal reflux.
7. Cereals allergy:
Suffered by children and adults alike, wheat allergy appears to be particularly associated with exercise-induced anaphylaxis. The more of a cereal (wheat, rye, barley, oats, maize or rice) we eat the more likely we are to suffer an allergy. Thus rice allergy is found more frequently in populations eating ethnic diets. Seed storage proteins (such as wheat gluten) and other proteins present in grain to protect it from attack by moulds and bacteria, have been found to be major allergens.
8. Penicillins allergy reaction:
Frequently manifestations are – rash, itching, urticaria and fever. Wheezing, angioneuritis, edema, serum sickness and exofolliative dermatitis are less common. Anaphylaxis is rare but may be fatal. Fear of anaphylactic shock has several restricted the use of injPnG in general practice. All form of penicillin (natural or semi synthetic) can cause allergy but it is more commonly seen after parenteral administration.
Penicillin produce hypersensitivity – urticaria, angioedema, bronchiospasm, anaphylaxis or serum sickness. If earlier reaction had been only a rash, penicillin may be given cautiously – often no untoward effect is seen. History of penicillin allergy must be elicited before injection it. A scratch test on intradermal test (with 2 – 10 u ) may be performed first. On occasion, his it has caused fatal anaphylaxis. Testing with benzyl-penicilloyl – polylysine is safer. However a negative intradermal test does not rule out delayed hypersensitivity. It should also be released that presence of antibodies to penicillin does not mean allergy to it, because practically everyone who receives penicillin develop antibodies to it.
For the development of antibodies, penicillin or a product of it (mostly penicilloyl moiety – major determent) act as a hapten. Topical use of penicillin is highly sensitizing (contact dermatitis and other reaction). Therefore, all topical preparation of penicillin (including eye ointment) have been banned, except for use in eye as solution in case of gonococcal opthalmia (9).

PATHOPHYSIOLOGY
A food allergy is an immunologic response to a food protein Food allergy is type 1 hypersensitivity reaction. Type I Hypersensitivity is characterised by excessive activation of mast cells and basophils by IgE, resulting in a systemic inflammatory response that can result in symptoms as benign as a runny nose, to life-threatening anaphylactic shock and death.
Exposure to an allergen activates B cells to form IgE secreting plasma cells . The secreted IgE Molecules bind to IgE specific Fc Receptor on mast cells. Second Exposure to allergen leads to cross linking of the bound IgE triggering release of pharmacologically active mediators vasoactive amines (11).
Immunoglobulin E (IgE):
The potent biological activity of IgE allowed it to be identified in serum despite its extremely low average serum concentration (0.3microgram/ml).IgE antibodies mediate the immediate hypersensitivity reaction that are responsible for the symptoms of hay fever, asthma, hives and anaphylactic shock. When the appropriate antigen was later injected at the same site, a wheal and flare reaction developed there .this reaction, called the p-k reaction was the basis for the first biological assay for IgE activity.
IgE binds to Fc receptors on the membranes of blood basophils and tissue mast cells, Cross-linkage or receptor bound IgE molecules by antigen (allergen) induces basophils and mast cells to translocation their granules to the plasma membrane and release their contents to the extracellular environment, a process known as degranulation. As a result, a variety of pharmacologically active mediators are released and given rise to allergic manifestations.
Fc Receptors:
Many cells feature membrane glycoproteins called Fc receptors (FcR) that have an affinity for the Fc portion of the antibody molecule. These receptors are essential for many of the bioligocal functions of antibodies. An FcR binds IgE like: The cytoplasmic domains of the chains of FcRI are associated with protein tyrosine kinase (PTKs). Crosslinkage of the FcRI receptors activates the associated PTKs, resulting in the phosphorylation of tyrosines within the ITAMs of the subunit as well as phosphorylation of residue on the subunit and on phospholipace C. These phosphyrylation events incduce the producton of a number of second messengers that mediate the process of degranulation.
Within 15 s after crosslinkage of FcRI, methylation of various membrane phospholipids is observed, resulting in an increase in membrane fluidity and the formation of Ca2+ channels. An increase of Ca2+ reaches a peak within 2 min. of FcRI crosslinkage. This increase is due both to the uptake of extracellular Ca2+ and to a release of Ca2+ from nitracellular stores in the endoplasmic reticulum. The Ca2+ increase eventually leads to the formation of arachidonic acid, which is converted into two classes of potent mediators: prostaglandins and leukotrienes. The increase of Ca2+ also promotes the assembly of microtubules and the contraction of microfilaments, both of which are necessary for the movement of granules to the plasma membrane. The importance of the Ca2+ increase in mast-cell degranulation is highlighted by the use of drugs, such as disodium cromoglycate (cromolyn sodium), that block this influx as a treatment for allergies (11).
Concomitant with phospholipid methylation and Ca2+ increase, there is a transient increase in the activity of membrane-bound adenylate cyclase, with a rapid peak of its reaction product, cyclic adenosine monophosphate (cAMP), reached about one min. after crosslinkage of FcRI. The effect of cAMP is exerted through the activation of cAMP-dependent protein kinases, which phosphorylate proteins on the granule membrane, thereby changing the permeability of the granules to water and Ca2+. The consequent swelling of the granules facilitates their fusion with the plasma membrane, releasing their contents. The increase in cAMP is transient and is followed by a drop in cAMP to levels below baseline. This drop in cAMP appears to be necessary for degranulation to proceed; when cAMP levels are increased by certain drugs, the degranulation process is blocked. Histamines are the main mediator of food allergy (11).
IgE Crosslinkage Initiates Degranulation
The biochemical events the mediate degranulaton of mast cells and blood basophils have many features in common. For simplicity, this section presents a general overview of mast-cell degranulation mechanisms without calling attention to the slight differences between mast cells and basophils. Although mast-cell degranulation generally is initiated by allergen crosslinkage of bound IgE, a number of other stimuli can also initiate the process, including the anaphylatoxins (C3a, C4a, and C5a) and various drugs. This section focuses on the biochemical events that follow allergen crosslinkage of bound IgE as follows
Receptor Crosslinkage
IgE-mediated degranulation begins when an allergen cross links IgE that is bound (fixed) to the Fc receptor on the surface of a mast cell or basophil. In itself, the binding of IgE to FcRI apparently has no effect on a target cell. It is only after allergen crosslinks the fixed IgE-receptor complex that degranulation proceeds. The importance of crosslinkage is indicated by the inability of monovalent allergens, which cannot crosslink the fixed IgE, to trigger degranulation.
Genetic basis
There is much evidence to support the genetic basis of allergy, as allergic parents are more likely to have allergic children, and their allergies are likely to be stronger than those from non-allergic parents. However some allergies are not consistent along genealogies with parents being allergic to peanuts, but having children allergic to ragweed, or siblings not sharing the same allergens. Ethnicity has also been shown to play a role in some allergies. Interestingly, in regard to asthma, it has been suggested that different genetic loci are responsible for asthma in people of Caucasian, Hispanic, Asian, and African origins. It has also been suggested that there are both general atopy genes and tissue-specific allergy genes that target the allergic response to specific mucosal tissues. Potential disease associated alleles include both coding region variation and SNPs. Caucasian people seem to have the most asthma (11).
DIAGNOSIS OF FOOD ALLERGY
Diagnosis of food allergy is based on clinical history, skin prick tests, and laboratory tests to detect serum-food specific IgE, elimination diets and challenges (12).
1. History: The physician interviews the patient to determine if the facts are consistent with a food allergy. . The doctor asks such questions as: What was the timing of the reaction? Did the reaction come on quickly, usually within an hour after eating the food? Was treatment for allergy successful? For example, if hives stem from a food allergy, antihistamines should relieve them. Is the reaction always associated with a certain food? Did anyone else get sick? For example, if the person has eaten fish contaminated with histamine, everyone who ate the fish should be sick. In an allergic reaction, however, only the person allergic to the fish becomes ill.
2. Dietary Diary: To keep a record of the contents of each meal and whether reactions occurred that are consistent with allergy. The dietary diary provides more details than the oral history, so that the doctor and patient can better determine if there is a consistent relationship between a food and the allergic reactions.
3. Elimination Diet: The next step is an elimination diet. The patient does not eat a food suspected of causing the allergy, for example, eggs, and substitutes another food, in this instance, another source of protein. If after the patient removes the food, the symptoms go away, the doctor almost always can make a diagnosis of food allergy. This technique is also not suitable if the allergic reactions have been infrequent13.
4. Skin Prick Tests: The skin prick is easy to do and results are available in minutes. Different allergists may use different devices for skin prick testing. Some use a bifurcated needle, which looks like a fork with two prongs. Others use a multi-test, which may look like a small board with several pins sticking out of it (14).
In a scratch-the-skin test, a dilute extract of the suspected food is placed on the skin of the forearm or back. This portion of the skin then is scratched with a needle and observed for swelling or redness, which would signify a local allergic reaction to the food. A positive scratch test indicates that the patient has the IgE that is specific for the food being tested on the skins mast cells. In some highly allergic patients, however, especially if they have had anaphylactic reactions, skin tests should not be done because they could provoke another dangerous reaction. Skin tests also cannot be done in patients with extensive eczema (13).
It is good for quickly learning if a person is allergic to a particular food or not, because it detects allergic antibodies known as IgE. Skin tests cannot predict if a reaction would occur or what kind of reaction might occur if a person ingests that particular allergen. They can however confirm an allergy in light of a patients history of reactions to a particular food. This almost painless procedure allows the tested protein to interact with food-specific IgE on the surface of skin mast cells (12).
A device, such as a lancet, plastic probe or tip of a small gauge needle, is pressed through a commercial extract of food and a positive (histamine) and negative (saline-glycerine) controls into the epidermis. Allergens eliciting within 15 min a wheal at least 3 mm larger than that produced by the negative control are considered positive, indicating the possibility that the patients have symptomatic reactivity to the specific food, with strongly positive results implying a greater likelihood of clinical reactivity. When evaluating allergy to fruits and vegetables, commercially prepared extracts are often inadequate because they are prone to degradation, and therefore the fresh food might be used for prick-by-prick test. A number of investigators have examined the use of the atopy patch test in addition to skin prick test for the diagnosis of non-IgE-mediated food allergy, with delayed reactions to food, but at this time, there are no standardized reagents or methods of application and interpretation. Thus, its diagnostic accuracy remains still controversial, especially in older children (12).
In Eosinophilic gastroenteritis associated with food allergy and bronchial asthma. A skin prick test and RAST (radioallergosorbent test) to causative food allergens showed a negative result. A fiber-optic endoscopic biopsy from the gastric mucosa showed an intense eosinophilic infiltration. The fiber-optic endoscopic biopsy might be needed to identify coexisting EG if an allergic patient with blood eosinophilia complains of severe gastrointestinal symptoms (14).
5. Blood Tests:- Blood tests such as the RAST (radioallergosorbent test) and the ELISA (enzyme-linked immunosorbent assay). These tests measure the presence of food-specific IgE antibodies in the blood of patients, but they cost more than skin tests, and the results are not available immediately (13).
In RAST, blood is drawn and sent to a lab for testing to determine predictive values for certain foods. These predictive values can be compared to the RAST blood test results. If a persons RAST score is higher than the predictive value for that food, then there is over a 95% chance the person will have an allergic reaction (limited to rash and anaphylaxis reactions) if they ingest that food. Currently, predictive values are available for the following foods: milk, egg, peanut, fish, soy, and wheat. Blood tests allow for hundreds of allergens to be screened from a single sample, and cover food allergies as well as inhalants. However, non-IgE mediated allergies cannot be detected by this method. Blood testing methodologies currently available that can measure antibodies of IgG are not acceptable as a method of allergy evaluation. IgG-type anitbodies are not implicated in food allergy reactions15.
(A) In vitro IgE food allergen-specific assays:-
The in vitro IgE assay is less sensitive, however, then the epicutaneous skin test. The in vitro IgE assay uses the same commercial food extract16.
1. Redioallergosorbeny (RAST):- The first test using this new technology was the redioallergosorbent test (RAST). The first step requires coupling the allergen (e.g., cows milk protein) to solid support (e.g., a paper disk). When this disk is placed in a test tube with patient serum, the coupling of antibodies directed against milk or that were anti- cows milk in nature (this includes IgE, M, A, or E class) occurs. After washing, the third step is the addition of radio-labelled (I125) rabbit antihuman IgE antibodies .these antibodies will attach themselves only to the IgE antimilk antibodies( which in turn are coupled to the cows milk protein in the paper disk). Then the amount of radioactivity can be measured, and thus the amount of cows milk-allergen-anti-IgE cows milk antibody can be quantities (16).
2. Enzyme-Linked Immunosorbent Assay (ELISA) :- This is more recent type of test, but is just like the RAST test except for two factors. First, allergen is usually coupled to the inner surface of small plastic walls (into which the patients serum and other ingredients of the test are placed). The second, and more important, difference is the final quantitative factor. In the ELISA system, an enzyme coupled to the antihuman IgE denatures a substrate, which either change color or becomes fluorescent. Then the amount of color or fluorescence can be measured(16).
(B) The production of interferon-gamma in response to a major peanut allergy, Ara h II correlates with serum levels of IgE anti-Ara h II.
To examine the potential role of T cells in the pathogenesis of peanut allergy. Peripheral blood mononuclear cells (PBMCs) from patients with peanut allergy, patients with asthma, and nonatopic normal control subjects were assessed for proliferation after stimulation with a 17 kd major peanut allergen (Ara h II), ovalbumin, casein, soy, and Candida albicans. That Ara h II and C. albicans induced significantly higher levels of proliferation than ovalbumin, casein, and soy. Because interferon-gamma (IFN-gamma) and interleukin-4 (IL-4) play critical roles in IgE regulation, the production of these cytokines after stimulation with C. albicans and Ara h II. C. albicans stimulated similar levels of IFN-gamma in all three study groups. In contrast, after stimulation with Ara h II, culture supernatants from PBMCs of subjects with peanut allergy contained significantly lower levels of IFN-gamma than did the PBMCs of the two control groups (p = 0.02). More important, there was a significant (p = 0.05) inverse correlation between the serum IgE anti-Ara h II levels and IFN-gamma production by PBMCs from the respective peanut-allergic patients. IL-4 protein was not detected in culture supernatants of PBMCs stimulated with Ara h II. However, amplification of cytokine gene transcripts by polymerase chain reaction did demonstrate IL-4 expression in Ara h II-stimulated PBMCs from both patients with peanut allergy and control subjects. Conclusion is that the level of IFN-gamma production in response to Ara h II may be an important factor in determining the development of peanut-specific IgE responses (17).
(6). Other Laboratory Tests:- When evaluating patients with gastrointestinal symptoms, suspecting food hypersensitivities, a number of other standard laboratory studies might be useful.
